The Annual Report should be submitted by all chapters and colonies on April 15. Information in the report will be used to determine awards and citations; insurance discounts and surcharges (colonies will not receive surcharges); and chapter accreditation. Scores will be posted on the National website, included in the Tomahawk Magazine and distributed to both alumni and Greek Advisors.

The Annual Report may require several officers to be involved in completing this form and documentation to be provided. It is recommended that you review the Annual Report regularly and begin working on its completion at least several weeks prior to the April 15 deadline.
